Purpose of the processing:

OLEA MEDICAL wishes to help its partners in the In Extremis project to certify that the participating physicians comply with the criteria for inclusion in the project.

The answers to this questionnaire are subject to automated processing giving rise to a result (Admitted/Not Admitted).

The collected data are not processed for commercial purposes.

Recorded data

Name, first name, phone number, email address, answers to questionnaires, analysis of responses.

Data recipients:

The data are accessible by Olea Medical staff involved in the In Extremis project. The data are also transmitted to the coordinators of the In Extremis project (European Union, United States).

Duration of data retention

The data are kept for the duration of the project and for 10 more years following the term of the project, in order to be able to justify to the competent authorities the modalities and progress of the study.

Rights of individuals:

You have the right to access your personal data or request their erasure. You also have the rights of objection, rectification, processing limitation and of portability of your data. To exercise these rights or if you have any questions about the processing of your data in this system, you can contact our Data Protection Officer (DPO).
